Impact Concussion Baseline Assessments
SHACC impact concussion consent form. Impact Baseline Testing is offered to all SHACC eligible students who attend Sault Area High School and/or Career Center, Sault Area Middle School, and Malcolm School. The testing is free. If concussion follow-up is needed insurance will be billed. However, all services at SHACC are subject to a sliding fee based on the student's income. No student is denied service based on inability to pay.
